As a student athlete, he earned a track scholarship while balancing the demands of studying, training, and working part-time installing metal roof panels in the Arizona heat. His determination led him to pursue a degree at the Arizona School of Dentistry & Oral Health, where he graduated and then gained practical experience in a corporate dental setting in Mesa.

Dr. Donald A. Eberhart grew up in the suburbs of Southern California and earned his Bachelor of Science in Biology, with minors in Psychology and Art History, from the University of California, San Diego. After completing his degree, he took some time to travel across Europe before attending the University of the Pacific School of Dentistry in San Francisco, where he graduated with honors. After spending most of his life in California, Dr. Eberhart made the decision to relocate to Arizona and begin his dental career in the Grand Canyon State.

Dr. Jaime A. Tobon is a native of Miami, FL. He is a former Sergeant and a veteran of the US Army. He received an honorable discharge after serving tours in Honduras, Germany, and Fort Riley, Kansas. He earned his Bachelor of Science in Biology from Kansas State University before earning his degree from Missouri School of Dentistry in Kansas City. He also attended an oral surgery externships at the Truman Medical Center in Kansas City, and at Banner Good Samaritan Hospital in Phoenix while in dental school.

Dr. Tobon completed an Advanced Education in General Dentistry (AEGD) residency after graduation in order to elevate his training in advanced oral surgery procedures such as dental implant surgery and endodontics. He is committed to dental excellence, which is why he has training in multiple aspect of dentistry including TMD therapy and computer-guided implant placement. He is a fellow and an active member of the International Congress of Oral Implantologists (ICOI).

Dr. Sharon Bader was born in Trinidad, West Indies. In 1988, she earned her Bachelor of Science form the University of Toronto. In 1996, Dr. Bader graduated from the University of Michigan in Ann Arbor MI. She then enhanced her training by attending a residency in Advanced Education in General Dentistry at the University of Michigan.

Dr. Bader has been a practicing dentist ever since, and she completes over 50 hours of post-graduate continuing education annually to stay up to date with current dental science, including advancements in techniques, materials, and dental implants.

During her nine years of active duty, Dr. Elizabeth had the opportunity to serve in diverse and impactful roles. She was the command dentist on a humanitarian mission to El Salvador, worked as a White House Social Aide to the President, and traveled globally as a combat medic, providing care to those who protect and serve our nation. After completing her military service, she returned to private practice and now serves as a managing partner at Arrowhead Family Dentistry. Dr. Elizabeth also continues to serve her country as a reservist in the United States Air Force.

A native of Phoenix, Dr. Elizabeth is thrilled to practice dentistry in the community where she grew up. I just had my first consult and surgery at Family & Cosmetic Dental in Phoenix, AZ. I am so amazed with Dr. Pulkrabek’s patience in explaining the procedure to me. He was able to look at my xrays from the exam and diagnose me quickly. Plus, the cost of his service is unmatched by any other dentist I’ve been to. But what really impressed me was that he only charged me half of what other dentists were going to charge for the same procedure, which saved me thousands of dollars! The other guy was going to charge me nearly $1k for just the anesthesia. The surgery went really smoothly and as planned! It didn’t hurt that much and the price he quoted me was on point. I healed quickly, too. The only thing I need to do is get a crown on the implant because his job was just to put the screw in. Now, I need to go to my usual dentist for the crown. They also cleaned my teeth at this office, and they did a great job. Two thumbs up from me!

My wife and I moved to Phoenix about a year ago, and we were nervous about finding a good dentist near me. We’ve tried three different places, but none of them were satisfactory. I only had to go in once for her to get the crown completely finished. There was almost no pain, the only thing that hurt a little bit was the novocaine needle. The crown looks great.
